Provision of quality respiratory and sleep diagnostic tests for inpatients and outpatients

The Respiratory and Sleep Medicine Department at the Bon Secours Hospital Dublin provides a comprehensive service diagnosing, treating, and caring for patients with a wide range of respiratory and sleep disorders.  Our 6 Respiratory and Sleep Medicine Consultants are supported by specialist nurses and respiratory physiologists and work closely with colleagues across the hospital including Diagnostic Imaging / X-rayPhysiotherapy and Endoscopy to provide a swift and complete service for all patients.  

The Rapid Assessment Respiratory Clinic operates within the department providing an innovative streamlined service for patients requiring timely expert respiratory opinion and investigations including post covid patients experiencing ongoing respiratory issues. Investigations are in most cases completed and evaluated in the same visit and an individualised action / care plan devised for patients.

Pulmonary Function Testing Lab

In 2022, the department opened a new state of the art Pulmonary Function Testing (PFT) facility including 4 PFT suites and offering a range of routine and specialised diagnostic tests. 

Spirometry with broncodilator response

The Department provides standard and specialised diagnostic services for indications including:

  • Evaluation and follow-up of respiratory symptoms and disease
  • Evaluation and follow-up of sleep disorders incl:
  • Obstructive Sleep Apnoea Syndrome and other sleep related conditions.
  • Quantification of the severity of known lung disease
  • Assessment of the potential effects or responses to environmental or occupational exposure

Respiratory Testing Service (GP referral)

The comprehensive range of investigations provided include:

  • Pulmonary Function Testing
  • Spirometry with bronchodilator response testing
  • Static lung volumes
  • Diffusing capacity
  • Body plethysmography (including thoracic gas volume and airways resistance)

 

Specialised Respiratory Tests (Respiratory Physician referral)

  • Methacholine / Mannitol inhalation tests
  • Cardiopulmonary exercise testing
  • Maximum respiratory pressures (MIP/MEP/SNIP)

Sleep Diagnostic Service (Consultant Respiratory/Sleep Physician referral only)

The department has 3 dedicated sleep study beds providing high quality overnight diagnostic testing to over 700 patients each year. All studies are conducted within the hospital on an inpatient basis and are non-invasive.  

  • Polysomnography (PSG) incl. diagnosis of sleep apnoea
  • CPAP titration studies
  • CPAP device fitting and patient education
  • Multiple Sleep Latency Testing (MSLT) 
  • Overnight Oximetry

  • Allergy Testing for a number of airbourne allergens. 
  • Fiberoptic Bronchoscopy service including transbronchial needle aspiration ( Endoscopy)
  • Radiology including Chest x-rays, High Resolution CT scan and CT Pulmonary Angiogram (Diagnostic Imaging)

Rapid Assessment Respiratory Clinic

 

For patients with a chronic cough, uncontrolled or decreased control of asthma or COPD, new diagnosis of COPD / Pulmonary fibrosis or those with decreased exercise tolerance, patients can be referred to our new Rapid Assessment Respiratory Clinic.

This innovative streamlined service for patients requiring expert respiratory opinion and investigations ensures that patients are seen quickly by a specialist consultant and respiratory team, investigations are completed and evaluated in the same visit and an individualised action / care plan devised for patients.

The service is delivered by Dr Emmet O Brien and Prof Ross Morgan . This clinic is supported by a team of Respiratory Physiologists who provide diagnostic testing for this service.

Bronchoscopy

A Bronchoscopy is a test in which a hollow, flexible tube is inserted through the mouth and down into the airways allowing the doctor to look at the lower airways including the larynx, trachea, bronchi, and bronchioles. During the course of the examination, biopsies may be taken and examined. The procedure is done under sedation / throat spray. You will be asleep for the test but not under general anaesthetic.  

The procedure takes place in the Endoscopy unit. On arrival in the unit, you will complete your registration and asked to change into a hospital gown. The doctor will complete a consent form and will ask for your signature giving your permission for the examination.

You will be given a local anaesthetic spray into your mouth and / or a mild sedative through a vein in your arm to make you sleepy and relaxed. A plastic guard will be placed in your mouth to protect your teeth and the doctor will then pass a scope through your mouth and into your airways. When the procedure is complete, you will be taken to the recovery area.

When you are returned to the ward following your procedure, you will need to rest for a short period of time. A nurse will monitor your temperature, breathing, blood pressure and pulse rate from time to time to ensure that you are comfortable.  When you are more awake you will be given water and something light to eat two hours after your procedure.

 

Test preparation:  It is important that you are fasting for this procedure. If your procedure is in the morning, you should fast from 12 midnight the night before incl. avoiding chewing gum. You may have clear fluids such as water up to two hours before. If your procedure is scheduled for the afternoon, you can have a light breakfast of tea and toast at 6am but nothing after.

Results:  Following the procedure, your consultant will discuss the results with you.  A copy of the results will also be sent your GP.

Diffusing capacity

Diffusing capacity of the lung measures the transfer of gas from air in the lung, to the red blood cells in lung blood vessels. It is part of a comprehensive series of tests (pulmonary function testing) to determine the overall ability of the lung to transport gas into and out of the blood.

To measure diffusion capacity, you breathe a harmless gas, called a tracer gas, for a very short time, often for only one breath. The concentration of the gas in the air you breathe out is measured. The difference in the amount of gas inhaled and exhaled measures how effectively gas travels from the lungs into the blood. This test allows the health care provider to estimate how well the lungs move oxygen from the air into the bloodstream.

Spirometry with bronchodilator response

Spirometry measures airflow. By measuring how much air you exhale, and how quickly you exhale, spirometry can evaluate a broad range of lung diseases. On occasion you may be required to take an inhaler and repeat this process to assess possible improvement.

Biography

Conor was educated in Blackrock College and graduated from UCD.  After completing a three year Fellowship in Stanford University in California, he returned to Ireland as Consultant Respiratory Physician in Connolly Hospital and Mater Hospital and Professor of Medicine in UCD.  In addition, he currently serves as Clinical Director. He is a Fellow of the College of Physicians in Ireland and Fellow of the College of Physicians in London and Fellow of the College of Chest Physicians (U.S.).

His post-doctoral thesis was awarded for his work in lung transplantation.

He has served on various government committees; for three terms on the Health Research Board and for three terms on the Council of the College of Physicians.

Conor has delivered over two hundred invited papers to international medical and scientific societies and published over one hundred papers in the peer reviewed international literature.

Biography

Ross Morgan graduated from UCD in 1996 and undertook specialist registrar
training in Respiratory Medicine in Ireland followed by a 3 year fellowship in Pulmonary and Critical Care at Boston University. He has been a Consultant Respiratory Physician at Beaumont Hospital since 2007 and is a Fellow of the RCPI and of the American College of Chest Physicians. He is a council member of the Irish Thoracic Society and Chair of the Lung cancer group at the NCCP. He is also chairman of ASH Ireland.

His clinical and research interests are in lung cancer, pleural disease, airway inflammation and bronchoscopy.

Biography

Dr Emmet O’Brien graduated from University College Cork in 2006, and completed Higher Specialist Training in Respiratory Medicine in 2016. He was awarded a PhD from RCSI for his work in translational respiratory medicine and has authored several publications and book chapters. Dr O’Brien completed his fellowship training in Pulmonary and Critical Care Medicine in 2018 at the University of Pittsburgh Medical Centre in Pittsburgh, USA, and was a practicing faculty member there before returning to Ireland in 2019.

In addition to working as part of our respiratory team at the Bon Secours Hospital Dublin, Dr O’Brien works as a Consultant Respiratory Physician at Beaumont Hospital and is an Honorary Clinical Lecturer in Medicine at the RCSI, where he is active in both undergraduate and postgraduate medical education.

Dr O’Brien is clinically experienced in all areas of general respiratory and sleep medicine, with sub-specialist interests that include lung cancer, pulmonary nodules, COPD, sleep medicine, and advanced diagnostic bronchoscopy including EBUS.
