A new literature review and clinical update on heart failure with preserved ejection fraction (HFpEF), a rapidly evolving area of cardiology, was recently published in the Medical Independent.
Authored by Professor Dr Ali Aldujeli, Consultant Interventional Cardiologist at Bon Secours Hospital Limerick, the article provides a comprehensive overview of current research into the diagnosis, management and evidence-based treatment of HFpEF.
HFpEF is a specific type of heart failure that has emerged as one of the most significant challenges in cardiology and cardiovascular medicine in recent years. It now represents over half of all heart failure cases worldwide.
Linked to an ageing population and the growing burden of risk factors and associated conditions such as diabetes and hypertension, the diagnosis and management of HFpEF remains particularly nuanced.
Exploring recent developments in this area, Prof. Dr Aldujeli outlines how HFpEF has moved from what was previously described as a “therapeutic wasteland”, with very limited treatment options, towards a more practical and structured approach to care. This evolving evidence-based model, often described as a “multi-pillar” approach, brings together pharmacological therapy with proactive management of underlying risk factors and comorbidities.
Key treatment options now include SGLT2 inhibitors, with strong evidence supporting their use in patients with HFpEF. Other therapies, including Finerenone, a non-steroidal mineralocorticoid receptor antagonist, and newer options such as GLP-1 receptor agonists, are also broadening the treatment landscape, particularly for patients with metabolic risk factors.
Prof. Dr Aldujeli also highlights the ongoing challenges in diagnosing HFpEF, including the importance of structured scoring systems and advanced imaging modalities in supporting accurate diagnosis. Ongoing clinical trials and continuing research are expected to further refine diagnostic pathways and therapeutic strategies.
These developments mark an important step forward in the clinical understanding and management of patients with heart failure with preserved ejection fraction.
